In 2023, new diagnostic criteria for mild traumatic brain injury (mTBI) were published. These criteria depart from past criteria in several ways, including by allowing subjective symptoms and acute examination findings to be sufficient for diagnosis.

The course of recovery from mTBI is reasonably well understood. Injured athletes and trauma patients perform more poorly on neuropsychological tests in the initial days and up to the first month following the injury. However, neuropsychological deficits typically are not seen in athletes after 1-4 weeks and in trauma patients after 1-3 months following injury. Early intervention, including psychoeducation on positive prognosis, can be helpful for reducing the risk of prolonged recovery and/or iatrogenic and expectancy effects.

A recent meta-analysis concluded that somewhere between one in three and one in six adults who present to an emergency department or trauma center with mTBI report at least mild severity persistent symptoms 3-6 months later.

People who sustain mTBIs are at increased risk for developing depression. There is growing evidence that exercise, vestibular rehabilitation, and psychological interventions can be helpful for reducing persistent symptoms following mTBI.
